Connect for Life and Annuity Fee Schedule
$725 — Early Registration, open until April 20, 5:00 p.m. ET
$850 — Regular Registration, April 21 through the conference

Full conference registration fees cover admission to one Sunday recreational activity (while available), all sessions and CSC-hosted evening events through Thursday, as well as daily breakfast, lunch and snacks as provided.

Spouse/Guest Fee Schedule
$250 — Spouse/Guest Registration (one adult guest per attendee)

CSC’s Connect for Life and Annuity guest fee covers admission to one Sunday recreational activity (while available) and all CSC-hosted evening social events through Thursday. Breakfast and lunch functions are not included in the guest fee.

Confirmation
Your registration is not valid until you have successfully completed the entire process and received a printable confirmation page as well as an email confirmation.

Your hotel room request is protected in our block of rooms. Your reservation will be assigned and a hotel confirmation will be mailed to you approximately 2 weeks prior to the event.

Changes and Cancellations

Conference and Guest Registration
Changes can be made through the event registration website until Monday, June 8, 2012. After this date, you will be able to make changes at CSC's onsite registration desk upon your arrival.

Hotel Reservations
Make any changes to your hotel reservation through the event registration website until June 8, 2012. Beginning June 9, please make hotel changes by calling the hotel directly at 504.648.1200.

NOTE: Cancellations must be made to your hotel reservation a minimum of 72 hours in advance or you will be charged for one night's room rate, plus tax. There is an early checkout fee of $75. Guests wishing to avoid the early checkout fee of $75 should advise the hotel at or before check-in of any change in planned length of stay.

Sunday Recreational Activities
Changes can be made through the event registration website until June 8, 2012. After this date, you will be able to change your Sunday itinerary at CSC's onsite registration desk upon your arrival. Please note that additions are made only if space is available.

Cancellations
If you need to make a cancellation send an e-mail to lifeandannuity@attendeenet.com. A processing fee of $100 will be charged for all Connect conference and guest cancellations.
